Select your level of cover from $25,000 up to $1,125,000. The maximum allowable level of cover is based on a life insured's age, gross annual income and other life insurance held at the time of application:
- Ages 18-44 can apply for up to $1,125,000
- Ages 45-54 can apply for up to $625,000
- Ages 55-59 can apply for up to $250,000

No, however if you wish for your partner to be covered they can call back later and apply for a separate policy. Unfortunately, neither you nor your partner will receive the combined cover discount if your partner is covered under a separate policy. |
| Can I nominate a beneficiary to receive my benefits? |

No, you cannot nominate any beneficiaries as there is no Life Benefit payable under ANZ Critical Illness Cover. You as the policy owner are the only person who is entitled to receive any benefits under this policy in the event that a life insured suffers an insured event as described in the ANZ Critical Illness Cover Product Disclosure Statement and Policy. |
