Setting up your SMSF with ANZ Self Managed Super is simple, all you need to do is fill out the appropriate application form and we'll help you get started.
Individuals who do not have a SMSF will need to follow the establishing a new fund instructions.
Individuals who have an existing SMSF with another provider will need to follow the transferring an existing fund instructions.
Important information
If your life insurance is currently covered by your existing commercial superannuation fund, you may need to replace this when you move to a SMSF. You should check with your current provider regarding your life insurance and ensure that you have sufficient cover prior to transferring any balance.

Establishing a new fund
Complete the ANZ Self Managed Super application form - new fund (PDF 204kB)
Mail your completed application form to:
ANZ Self Managed Super
Locked Bag 5
Forest Hill VIC 3131.
You will be sent all documentation required to establish your SMSF.
Complete, sign and return documentation
- Sign all relevant documentation and return the completed forms to ANZ Self Managed Super.
- The necessary forms will be lodged on your behalf.
Welcome kit
In your ANZ Self Managed Super welcome kit you will receive:
- relevant ATO documents (TFN, ABN etc).
- cash management account (ANZ V2 PLUS) number.
- login username for ANZ Self Managed Super (your password will be sent separately) so that you can access reports and view your fund's progress online.
Transferring an existing fund
Complete the ANZ Self Managed Super application form - existing fund transfer (PDF 196kB)
Mail your completed application form to:
