ANZ Smart Choice Super and Pension Insurance premiums
The following table shows:
- the level of cover you receive,
- the corresponding current annual premium rates based on your age, smoker/non-smoker status and your gender, and in the case of Total and Permanent Disability (TPD), your occupation.

Your cover will commence when your account has received an initial contribution. Premiums are deducted from your respective ANZ Smart Choice account at the beginning of each month and are paid to the insurer.

Making a claim
In the event of Death or Total and Permanent Disability for an ANZ Smart Choice Super and Pension member who holds a policy for the applicable insurance, the applicable forms are available below.
Your insurance claim must be made through the Trustee (OnePath Custodians Pty Limited). The Insurer (OnePath Life Limited) will assess the claim and if accepted will pay the benefit to the Trustee who must approve the claim before any benefit is paid.
In the event of a Terminal illness, a terminal illness benefit may be paid if you become terminally ill whilst your Death cover is in force. You are regarded as terminally ill when you are diagnosed by two medical practitioners (one of whom is a specialist physician) as having a terminal illness, and you are not expected to live for more than 12 months from the date of written certification despite reasonable medical treatment. The terminal illness benefit is not a stand-alone benefit – it is an advanced payment of the Death benefit.

Before redirecting your super or moving your money into your ANZ Smart Choice Super account, you will need to consider whether you will incur any withdrawal fees, costs or lose any benefits when moving from your other fund. You will also need to consider whether moving your super from your current fund will affect any insurance cover you have in your other fund(s).

OnePath Custodians reserves the right to change insurer, or vary the benefits or premium rates from time to time.
